Introduction

Obstructive sleep apnea (OSA) and periodontitis are interconnected health conditions that significantly impact patients’ quality of life. Recent research published in *Dentistry Journal* (2025) by Lin et al. explores the efficacy of oral appliance therapy (OAT) using the myTAP oral appliance with a mouth shield in patients with mild to moderate periodontitis who also exhibit snoring and mouth breathing (MB) associated with OSA. This article highlights the positive outcomes of the myTAP appliance, demonstrating its potential as an adjunctive therapy for improving periodontal health and sleep respiration parameters.  

Study Overview

The study, titled “Effects of Oral Appliance Therapy with a Mouth Shield in Periodontitis Patients Who Snore: A Split-Mouth Randomized Controlled Trial,” was conducted at Texas A&M University College of Dentistry. It involved 14 adults with mild to moderate periodontitis (Stage I-II, Grade A-B) and confirmed OSA with MB. The trial employed a split-mouth design, where one side of the mouth received scaling and root planing (SRP) alongside OAT with the myTAP appliance, while the contralateral side received only superficial subgingival polishing. All participants used the myTAP oral appliance, which was fitted chairside and self-titrated, with mouth shields introduced at 8 weeks to address MB.    

Key Positive Outcomes

The study revealed several positive outcomes associated with the use of the myTAP oral appliance with a mouth shield, particularly in improving periodontal and sleep-related parameters.  
1. Reduction in Mouth Breathing
Mouth breathing is a known contributor to periodontal disease due to its association with reduced salivary flow and altered oral microbiome. The myTAP appliance significantly reduced MB, with the percentage of MB decreasing from a baseline median of 8.9% (interquartile range: 2.1–36.1%) to 1.9% (0.8–8.1%) at 12 weeks (p=0.022). This reduction suggests that the mouth shield component effectively promotes nasal breathing, potentially mitigating the drying of oral tissues and supporting a healthier oral environment.  
2. Improved Sleep Respiration Parameters
The myTAP appliance demonstrated significant improvements in sleep respiration metrics, critical for managing OSA: – **Respiratory Disturbance Index (RDI):** The overall RDI decreased from 22.1 events/hour (15.0–28.1) at baseline to 12.6 events/hour (5.1–17.6) at T3 (p=0.009). – **Supine RDI:** In the supine position, RDI reduced from 27.6 events/hour (21.2–39.0) to 12.7 events/hour (4.5–19.1) (p=0.002). – **Oxygen Desaturation Index (ODI):** ODI improved from 12.2 events/hour (8.0–22.5) to 8.2 events/hour (5.0–16.5) (p=0.025).   These improvements indicate that the myTAP appliance effectively reduces airway collapsibility and enhances oxygenation, addressing key OSA-related symptoms.  
3. Reduction in Periodontal Pathogens
The study found a significant reduction in *Porphyromonas gingivalis*, a key periodontal pathogen, in the SRP group using the myTAP appliance (p=0.041 at T3). While changes in other microbial flora were less pronounced, the reduction in *P. gingivalis* suggests that OAT, when combined with SRP, can positively influence the oral microbiome, potentially reducing periodontal inflammation.  
4. Decreased Bleeding on Probing
Bleeding on probing (BOP), a clinical indicator of periodontal inflammation, decreased significantly in the SRP-treated sites compared to the OAT-only group (p=0.028). This suggests that the myTAP appliance, when used adjunctively with SRP, enhances periodontal treatment outcomes by reducing gingival inflammation.  
5. No Adverse Effects on Periodontal Health
Importantly, the study found no negative impact of the myTAP appliance on periodontal health over the 12-week period. This supports the safety of OAT as an adjunctive therapy for patients with mild to moderate periodontitis, indicating that it does not exacerbate periodontal conditions.  

Clinical Implications

The myTAP oral appliance with a mouth shield offers a non-invasive, patient-friendly alternative to continuous positive airway pressure (CPAP) therapy for managing mild to moderate OSA. Its ability to reduce MB and improve sleep respiration parameters makes it a valuable tool for addressing OSA-related behaviors that contribute to periodontal disease. The significant reduction in *P. gingivalis* and BOP when combined with SRP highlights its potential as an adjunctive therapy in periodontal care, particularly for patients with OSA and MB.  

Limitations and Future Directions

While the study demonstrates promising results, its small sample size (n=14) and split-mouth design limit the generalizability of findings. The presence of 43% of subjects still exhibiting MB at 12 weeks suggests variability in response, possibly due to compliance or individual anatomical factors. Future research should involve larger sample sizes, parallel group designs, and direct measurements of inflammatory markers (e.g., salivary cytokines) to further elucidate the mechanisms by which OAT impacts periodontal health. Additionally, improved mouth shield designs could enhance MB reduction and treatment efficacy.  

Conclusion

The myTAP oral appliance with a mouth shield is a promising adjunctive therapy for patients with mild to moderate periodontitis and OSA. By significantly reducing mouth breathing, improving sleep respiration parameters, and decreasing key periodontal pathogens and inflammation, it offers a dual benefit in managing both periodontal and sleep-related conditions. These findings underscore the potential of OAT as a multidisciplinary approach to improving oral and systemic health in this patient population.
